About HCP
Horizon Care Pediatric is dedicated to rehabilitating the medical fragile community to include infants, children and adults within the comfort of their residence while ensuring the best quality service to our patients and families.
Our staff has the experience and compassion needed to provide care in a home environment without sacrificing quality or safety. The services are made available to meet specific needs of each patient. Our Clinical team is responsible for developing an individualized plan of care and working with your primary physician to insure appropriate care is rendered.
In order to accomplish our mission, our licensed and certified professionals are available 24 hours a day 7 days a week.
