Ateela Glenn is the visionary founder and CEO of Horizon Care Partners, a role inspired by her personal journey as a single mother of twins born fragile. The daily responsibilities of caring for her children were immensely taxing, and her primary commitment was their safety and well-being. During this challenging time, Ateela found strength in her spirituality, believing that everything would ultimately be okay. Her faith was bolstered by the invaluable support of community resources, which played a pivotal role in her family’s journey.

 

Motivated by her experiences, Ateela made the profound decision to join the Georgia Pediatric Program (GAPP) network. This commitment allowed her to serve countless parents facing similar challenges in locating essential resources for their fragile children. Her mission to provide support and guidance to these families has been both personally and professionally rewarding.

 

Ateela is dedicated to expanding Horizon Care Partners, with the goal of reaching and assisting as many families as possible. Her unwavering passion for empowering others and enhancing the quality of care for vulnerable children drives her efforts every day. Through her leadership, Ateela Glenn continues to make a meaningful impact in the lives of those she serves.

Tasha Blue is a highly experienced healthcare professional with over a decade of expertise in delivering exceptional patient care across diverse populations, including pediatrics and geriatrics. Her extensive background encompasses home health care and acute care settings, employing personalized, patient-centered approaches that prioritize the unique needs and preferences of each individual.

 

Demonstrating a strong commitment to excellence, Tasha integrates evidence-based practices into her care delivery, ensuring that every patient receives the highest standard of medical attention. Her empathetic demeanor and professionalism cultivate a supportive environment, enabling clients to achieve optimal health outcomes. Tasha’s dedication to enhancing patient well-being, combined with her proficiency in navigating complex healthcare systems, positions her as a trusted advocate for her patients.

 

As the youngest of six children and a devoted mother to one son, Tasha has a deep appreciation for family values. In her personal time, she enjoys gardening and farming, which further reflects her commitment to nurturing growth and well-being.

Vice President

Tywanna Hamilton is a devoted mother of four and grandmother of five. Recently, she faced profound loss with the passing of her only son and her ten-year-old grandson. Despite this heartache, Tywanna remains a resilient and inspiring figure.

 

With 31 years of nursing and clinical experience, she has dedicated her career to improving healthcare. Tywanna has spent 15 years in executive leadership roles, focusing on Operations and Business Development, and has devoted two decades to restructuring the healthcare system. Her diverse professional background includes a strong emphasis on gerontology, highlighting her commitment to enhancing the lives of older adults.

 

Tywanna’s life experiences have shaped her passion and dedication to healthcare, inspiring her to advocate for those in need.
